1. Any one (1) of the following valid ID's:
- Passport
- Driver’s License
- Social Security System ID (SSS)
- Unified Multi-purpose ID (UMID)
- PRC ID (front & back)
- Voter’s ID
- Postal ID
Metrobank accountholders may also submit any of the following IDs through account application via the website:
- Philhealth ID
- TIN Card
- Senior Citizen ID
- OFW or Overseas Workers Welfare Administration (OWWA) ID
- Integrated Department of Labor and Employment (iDOLE) Card
- Philippine National Police (PNP) Firearms License
- Integrated Bar of the Philippines ID
2. Three (3) Specimen Signatures. Sign three times on white paper and take a photo.
3. One (1) Proof of Address. Submit any document showing the preferred mailing address you indicated in the application form. Billing statement named under another person is fine.

4. Proof of Bank Account. Provide any of the following that shows your bank account number:

- Teller-validated deposit slip
- Bank statement/certification
- Screenshot of online/mobile banking account
- ATM card with bank account number (ATMs with only the debit card# not accepted)
- Passbook showing your name and account number

*Name on the bank account will be followed for married/maiden name
*Joint accounts require a joint bank account
*For In-Trust-For (ITF) accounts, the minor’s Metrobank Fun Savers/Spark account is accepted
5. Initial Deposit of ₱1,000 or $100. Upload a scanned copy or screenshot of the proof of transaction during application.
6. Other requirements:
a. Proof of Business / Self-employment. Provide any of the following:
i. Business/Mayor's Permit
ii. DTI/SEC/BIR Registration
iii. GIS/AFS
iv. BIR 2303 Form
b. Taxpayer Identification Number (TIN). How to get a Taxpayer Identification Number (TIN)?
c. SSS/GSIS is required for all employed. Exempted are foreign investors, OFWs, students, self-employed, unemployed, and retired, and GSIS-exempted individuals (type in 0):

- Members of the Judiciary and Constitutional Commissions who are covered by separated retirement laws;
- Contractual employees who have no employee-employer relationship with their agencies; and
- Uniformed members of the Armed Forces of the Philippines and the Philippine National Police, including the Bureau of Jail Management and Penology and the Bureau of Fire Protection. All government employees (including GSIS-exempted individuals) are required to submit Enhanced Due Diligence
